Serves primarily as a Staff Pharmacist, whose assignments involve complex problems of a wide range and highly specialized nature. Some of the duties involve direct patient contact.
The primary functions will be the interpretation of orders for appropriateness, preparing and dispensing of medicati! ons (inpatient and outpatient), monitoring and evaluation of drug thera! py and patient and staff education on medications.
The Staff Pharmacist must be a graduate of an accredited school of pharmacy and must be registered by a state or territory of the United States. Staff Pharmacist must demonstrate a high degree of knowledge in professional matters. Staff Pharmacist will be professionally competent. Professional competence is defined as possessing current professional knowledge and functioning in a manner consistent with demands of a changing profession and includes but not limited to unit dose, IV additives, pharmacokinetics, drug interactions and pharmacology. Will be required to communicate product information to patient, medical staff, nursing, and other hospital personnel.
POSITION FUNCTIONS:
CLINICAL: Provides consultation to physicians, nurses and other health care professionals concerning drug therapy; e valuates patientsÂ' medications and the patientsÂ' ability to understand medication instructions! , while providing oral and written counseling to clarify or reinforce the patients understanding; m onitors dosages of medication and intended therapy utilizing information and facts available; addresses physician pharmacy consults for aminoglycoside and vancomycin dosing; Addresses herbal/non-VA med consults and renal function consults; r eviews new orders for duplication, allergies and continuity of care from outpatient to inpatient and inpatient to outpatient; involvement in Quality Assurance Programs as to the instructions of immediate supervisor.
DISPENSING: Evaluate physician's orders for appropriateness based on medication profiles and known facts; dispenses medication in accordance with dosage requirements and fully capable of functioning at areas which trained in, i.e. Outpatient, Unit Dose, IV Admixtures; provides quality control analysis for dispensing activities of automated dispensing equipment or pharmacy technicians; t akes appropriate action ! on all medication orders including non-formulary, compounds, and out of! stock items; t akes responsibility for assisting technicians under his/her control or guidance; r esolves problems associated with the drug distribution process (inpatient and outpatient), such as missing doses, lost prescriptions, unusual usage patterns or inadequate availability of dosage forms and/or strengths of medication. Participates in medical center and department quality assurance programs (PMAI);
EDUCATION: Actively involved in teaching technicians-in-training or students; Shares information with other pharmacists which is necessary for daily operations of the pharmacy; Assumes responsibility for self-development in learning about new medications and current changes within the practice of pharmacy.
Physical Requirements: Moderate lifting 15-44 lbs; moderate carrying 15-44 lbs; reaching above shoulder; use of fingers, both hands required, walking up to 4 hours per day; standing up to 6 hours per day, repeated bending up to 2 hours,! ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and 20/40 in the other; ability to distinguish basic colors; ability to distinguish shades of colors; hearing (aid permitted). QUALIFICATIONS REQUIRED:
Basic Requirements:
- U.S. Citizenship
- Graduation from a ACPE accredited college or school of Pharmacy with a baccalaureate degree in pharmacy and/or a Doctor of Pharmacy (Pharm.D.) degree.
- Full, current and unrestricted pharmacist license
- English language proficiency
Specialized experience for GS-12 level : Requires one year of experience or completion of an ACPE! accredited Pharm.D. program.
NOTE: The full performanc! e level of this vacancy is GS-12. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-11 to GS-12 . The Professional Standards Board (a peer-review group) will review the selected candidate's education and experience and recommend the grade and step at which the candidate will be placed. The salary is based on the grade and step approved for the selected candidate.
