Duties:!
The Clinical Pharmacy Technician (Anticoagulation) primaryÂ's responsibility is the coordination and provision of clinical pharmacy services to the patients receiving anticoagulation therapy at the Cooper Drive and Leestown Divisions of the Lexington VA Medical Center. Specific clinical work assignments include participation in daily activities of pharmacy coordinated anticoagulation clinics, point of care laboratory testing, laboratory value monitoring, medical chart/action profile poly-pharmacy reviews, medication utilization monitoring, telephone logistical support, and supplemental patient education communication. On-site certification of clinical competency shall be completed prior to performing assigned clinical functions. This technician functions under the supervision of the Associate Chief, Clinical and Educational Programs, and will directly report to a Clinical Pharmacy Specialist.

Duties include but are not limited to:

  • Perform p! oint of care laboratory testing
  • Provide assistance with admission and discharge medication reconciliation pharmacy process as assigned
  • Routinely reviews computer medication profile for prescription and therapeutic duplication, prescription noncompliance, and allergies/adverse drug reactions
  • Reviews INR laboratory monitoring and identifies patients with lab values outside of normal limits
  • Provides supplemental reinforcement education for patients concerning operational issues or need for therapy compliance
  • Assists clinical pharmacy medication utilization evaluations and protocol-driven clinical pharmacist peer review
  • Documents clinical and operational activities in CPRS as assigned
  • Provides telephone triage and patient follow-up as assigned
  • Inputs refill and mail prescriptions into the computer after the prescriptions have been reviewed for appropriateness by a pharmacist
  • Provides assistance wit! h clinical pharmacy lectures and educational programs as assigned

Basic requirements:
GS-6:
Experience. One year experience at the next lower level, and must fully meet the KSAs at the next lower level.

Certification. PTCB certification is required at this grade level and above.

Assignments. Pharmacy technicians may be assigned to any area of the pharmacy including expanded distributive function assignments; for example assignment as team leaders for specific process improvement teams involving technical areas or performing more highly complex duties including preparation of oncology agents for clinical pharmacy specialists. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time.

Specialized Experience! : Inpatient and Outpatient experience in the clinical aspect of the hospital setting in addition to the basic requirements of one (1) year of specialized experience equivalent to at least the GS-5 grade level. Examples of specialized experience include but are not limited to: Ability to operate and care for automated dispensing devices and equipment; Knowledge of, and ability to perform aseptic technique; Knowledge of basic inventory procedures; Knowledge of pharmacy operations

In addition, candidate must demonstrate the following knowledge, skills , and abilities (KSAÂ's):

  • Ability to solve problems and make recommendations, e.g., troubleshoot operational problems and refer for appropriate action
  • Knowledge of the operation and care of automated equipment
  • Knowledge of, and ability to follow, written instructions for compounding with aseptic technique for hazardous materials such as oncology agents
  • Knowledge, and abili! ty to perform, compounding with aseptic technique for routine sterile p! roducts
  • Knowledge of policies and procedures for inventory management in specialized areas such as emergency carts
  • Knowledge of the computerized prescription process demonstrated by the ability to input prescriptions and medication orders

PHYSICAL REQUIREMENTS: Moderate lifting, 15-44 pounds; moderate carrying, 15-44 pounds; reaching above shoulder; use of fingers; both hands required; walking; standing; repeated bending for up to oen hour; ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; ability to distinguish basic and shades of color; hearing (aid permitted).
