he Manager of Clinical Pharmacy Services has responsibility for the daily provision of clinical pharmacy services subject to the review and approv! al by the Pharmacy Director.

EDUCATION & TRAINING:
Doctor of Pharmacy from an accredited school of pharmacy is required. Completion of a Pharmacy Practice (PGY-1) Residency or equivalent experience is required.

EXPERIENCE:
At least 5 years clinical hospital experience required. Experience with formulary management and coordination of clinical effectiveness programs required. Leadership experience in planning and setting direction for a department, team or program is preferred.

LICENSE & CERTIFICATION: Registered Pharmacist in Kentucky. Board certification is desired.

JOB PURPOSE/SUMMARY

The Manager of Clinical Pharmacy Services has responsibility for the daily provision of clinical pharmacy services subject to the review and approval by the Pharmacy Director. Collaborates with other Clinical Managers

Provides oversight of the medication formulary and ensures cost-effective utilization.
Designs and implements evidence-based medication use proces! ses that provide optimal patient outcomes consistent with currently accepted standards of practice.

Responsible for the coordination of Pharmacy & Therapeutics Committee
