VNA Nazareth Home Care (VNA), a service of Jewish Hospital & St. Mary’s HealthCare, has provided home care and wellness services to central Kentucky and southern Indiana communities for more than a century. Our innovative services and preventative health programs recognize the needs of our patients and promote the highest quality of life. VNA Nazareth Home Care has a team of registered nurses, social workers, companions, dieticians, pharmacists, aides and physical, occupational and speech therapists. The team provides care for Alzheimer’s disease, diabetes, mental health and ventilator use and offers programs for low vision, stroke management, wound management, better breathing, falls prevention, healthy heart and infusion therapy.
Required Experience:
Develops, implements, monitors, and modifies a plan of care though an interdisciplinary and collaborative team process in conjunction with the physician, the client, and his/her caregiver. Educates client and family about community support services; facilitates the goal of self-care by the client and his/her family. Monitors the plan to ensure the quality, quantity, timeliness, and outcome measures of services; provides periodic reassessment to assure that services are appropriate and cost effective and do not increase the client's dependence.
Minimum of 1 year clinical nursing experience required. Additional experience and/or certification for specialty areas.
Demonstrated ability to function autonomously within area of professional expertise. Professional certification in specialty preferred. Excellent oral and written skills. Ability to relate to general and professional public, all levels of agency staff, and patients. Thorough knowledge of VNA's patient care standards, payor requirements (i.e., Medicare Home Health Agency Manual HCFA Publication 11), and internal agency communications as they relate to issues of compliance.
Required Education:
Associate or Bachelor degree in nursing required. Kentucky RN license required.
