VNA Nazareth Home Care (VNA), a service of Jewish Hospital & St. Mary’s HealthCare, has provided home care and wellness services to central Kentucky and southern Indiana communities for more than a century. Our innovative services and preventative health programs recognize the needs of our patients and promote the highest quality of life. VNA Nazareth Home Care has a team of registered nurses, social workers, companions, dieticians, pharmacists, aides and physical, occupational and speech therapists. The team provides care for Alzheimer’s disease, diabetes, mental health and ventilator use and offers programs for low vision, stroke management, wound management, better breathing, falls prevention, healthy heart and infusion therapy.
Required Experience:
Evaluates documentation in the medical record related to reimbursement using VNA policies, procedures, and acceptable standards through monitoring mechanisms, such as audits and utilization review. Serves as a consultant to service staff regarding reimbursement and serviced issues. Performs periodic clinical record review, and utilizes reimbursement guidelines in completion of certification/re-certification and Medicare requests for medical information and denials. Uses personal computer system with Delta "online" interface for accessing, entering and processing VNA patient data. Performs job duties in a responsible manner and interacts with co-workers to create interpersonal relationships wich result in effective and efficient teamwork.
Minimum of 2 years of nursing experience required, preferably in community health nursing.
Knowledge of community health services. Ability to perform clinical documentation review. Excellent organizational and time management skills. Excellent oral and written communications. Ability to relate to the general public and all levels of agency staff. Skills with personal computer for word processing and data analysis preferred. Ability to function effectively as a member of a team that is responsible for a whole process. Interpersonal skills that facilitate inter- and intra-team/departmental communication.
Required Education:
Associate degree or diploma in nursing required. Kentucky Registered Nurse or Licensed Practical Nurse license.
