Pharmacy - Outpatient Oncology
Responsibilities
Responsible for the operations of the Outpatient ! Oncology pharmacy including daily operations, policies and procedures, Quality Assurance programs to resolve and identify problems. Drug security and ensuring all staff operating within the pharmacy is properly licensed trained and have documented competencies to function within their defined roles within the pharmacy. This includes scheduling, supervising and managing day to day operations of the pharmacy. Making or filing of any reports required by BHP or as required by state or federal laws and regulations. Responding to the KBOP regarding any identified violations or deficiencies and filing any report of loss to the appropriate authorities. Does at least annual job performance evaluations on all staff working with the Oncology Pharmacy and ensures operational and cognitive competence of all staff working with oncology drugs. Interpret and review each medication order received for correct dosage, schedule, allergies, interactions, contraindications and therapeutic duplic! ations. Evaluate, prepare and dispense total parenteral nutrit! ion orders. Evaluate, prepare and dispense chemotherapy admixture orders. Enter orders into the computer system. Check for proper order entry of IV and oral medications into the computer system by technicians. Check and observe IV technician preparation and labeling of intravenous admixtures. Check all oral medications selected and prepared by technicians and support personnel prior to dispensing for patient administration. Prepare and label prescriptions for take home medications. Compound inpatient drugs and outpatient prescriptions that require individual calculations. Counsel patients concerning prescriptions ¿ Includes IV therapy outpatient. Answer drug information questions for the medical staff, nursing staff and the public. Provide services for drug information. Dispense narcotics to nursing stations and maintains records of controlled substances and narcotics. Answer telephone and handle drug-related problems with health care professionals. Identify patients¿ hom! e medications and notify nursing personnel when okay to administer. Check expiration dates and restocks emergency drugs on code carts and kits. Check patient medication trays that are filled by technicians and assist in filling patient medication trays if needed. Work directly with staff for best drug utilization. Be available for advising physicians on proper medication selection for individual patients. Participate in maintenance of therapeutic drug monitoring program. Work on medication uses evaluation data collection. Help reduce cost and length of stay for patients by proper selection and utilization of medications. Follow hospital and departmental policies and procedures.
Qualifications
Must have a BS or Doctor of Pharmacy Degree, with current licensure by the Kentucky Board of Pharmacy. ACLS and oncology / chemo experience preferred.
