The Pharmacy Intern will report to the Pharmacist in Charge (PIC) on all matters related to the practice site, pati! ents, and pharmacy operations. The Intern is responsible for conducting Comprehensive Medication Reviews (CMR) and/or other Medication Therapy Management (MTM) initiatives as assigned.
The Pharmacy Intern will:
- Serve as a liaison to other healthcare practitioners in providing quality clinical pharmacy services in the respective ambulatory clinic setting.
- Carry out clinical services, whether in person or remotely, for all clients of Eumedics Health.
- Actively participate in the development/implementation/expansion of clinical initiatives.
- Provide education to patients and/or family regarding medications, devices and/or disease states, when warranted.
- Perform timely and appropriate clinical interventions to ensure optimal delivery of quality medication management.
- Document clinical notes completely and accurately in patient medical record in compliance with policies and standards.
- Document inter! ventions and track outcomes in a timely manner.
- Compl! y with all collaborative practice agreement requirements.
REQUIRED: - Kentucky Pharmacy Intern license.
- Completion of 2 years PharmD coursework at accredited College of Pharmacy.
- Excellent communication skills with patients and health care providers.
- Competency with technology including computers and mainstream software programs
- Prior experience with MTM.
