The Manager of Clinical Pharmacy Services has responsibility for the daily provision of clinical pharmacy services subject to the review and approval by the Pha! rmacy Director. Collaborates with other Clinical Managers in KentuckyOne Health and works closely toward integration with the KentuckyOne Health campus which is comprised of two (2) downtown facilities. Provides oversight of the medication formulary and ensures cost-effective utilization. Designs and implements evidence-based medication use processes that provide optimal patient outcomes consistent with currently accepted standards of practice. Responsible for the coordination of Pharmacy & Therapeutics Committee activities, including serving as Secretary of that Committee and in collaboration with the downtown KentuckyOne Health campus facilities. Collaborates with the Manager of Inpatient Pharmacy Services, to supervise and evaluate inpatient pharmacy activities to assure that medication orders are properly ordered, dispensed, administered and monitored in compliance with all state and federal regulatory and accrediting agencies. Works in conjunction with the Manager of I! npatient Services to co-ordinate inpatient pharmacy services a! nd process flow. Collaborates with other pharmacy managers, clinical pharmacy specialists and other clinical staff to plan performance improvement activities and medication safety actions. The position requires the ability to communicate and collaborate effectively with patients, physicians, nurses, pharmacists and allied health professionals from all clinical disciplines regarding activities related to the improvement of medication use. Evaluates the performance of all clinical pharmacists and provides ongoing feedback. Oversees and participates in educational activities of the department and in the development of pharmacy residents and students. Ensures that all activities are consistent with overall hospital objectives. The position demonstrates competence to provide pharmaceutical care for neonatal (birth-discharge), pediatric (birth to 11 years), adolescent (12-18), adult (19-65), and geriatric (66-over) patients.
1. Provides oversight in planning, development! , maintenance, and ongoing review of a formulary that promotes rational, cost-effective drug therapy across the organization. Ensures that formulary changes are communicated to appropriate providers and staff in a timely manner. Works toward integration with KentuckyOne Health downtown campus.
2. Supervises all aspects of ongoing pharmaceutical care to include organizational structure and staffing levels, as well as the performance of clinically based services such as patient rounds, clinical assessments, therapeutic drug monitoring, pharmacokinetic dosing, nutritional support, antimicrobial surveillance, etc.
3. Participates in the development and implementation of clinical pharmacy goals that are in alignment with the organization's mission, values and strategic plan.
4. Collaborates with pharmacy management team and provides over-sight in the planning, development and implementation of departmental quality improvement efforts to ensure positive! outcomes.
5. Provides leadership to ensure successful complia! nce with all federal, state and local regulatory standards. Ensures that all pharmacy staff maintains appropriate credentials on file.
6. Coordinates the development, implementation, maintenance, and monitoring of departmental and/or organizational policies. Works toward integration with KentuckyOne Health downtown campus.
7. Works to promote compliance with established policies within the department of Pharmacy and the Pharmacy & Therapeutics Committee.
8. Provides advice and guidance to medical staff and other clinical providers on the provision of evidence-based medication therapies and protocols with regards to age-specific populations.
9. Mentors staff in providing information to patients and patients' family members on safe and effective medication use.
10. Communicates with medical, nursing, pharmacy and ancillary staff in a positive and effective manner. Communicates and collaborates with hospital leaders and staff to ens! ure timely and accurate exchange of information across the organization, as well as KentuckyOne Health.
11. Plans, develops and implements or modifies clinical services to meet new or unmet needs. Collaborates with management team and KentuckyOne Health downtown campus pharmacy managers to resolve all identified problems and adopt preventative actions. Seeks external funding sources as needs arises.
12. Participates or provides oversight of participation in hospital standing and ad-hoc groups concerned with medication utilization and safety to define present and future needs and to assist in problem resolution.
13. Develops positive relationships in satisfying the needs of internal and external stakeholders.
14. Provides monthly and/or periodic reports to administration containing qualitative and/or quantitative data.
15. Oversees the recruitment, orientation, and evaluation of quality staff. Serves as a mentor to pharmacy staff,! aiding in the development of their clinical skills.
16. Overs! ees, develops and participates in educational endeavors involving medical, nursing, pharmacy, and ancillary staff.
17. Assists the Pharmacy Director with timely capital budget proposals, upon request; maintains revenue and expense within target goal.
18. Supports the mission of the Department of Pharmacy in the training and guidance of pharmacy students and residents. Serves as preceptor in the area of specialty. Attends and participates in education-related committee meetings. Ensures that evaluations are appropriate and that all accrediting standards are met.
19. Supports drug research conducted by the institution. In collaboration with the Manager of Drug Policy and Research, helps to ensure optimal handling of investigational agents and appropriate documentation.
19. Maintaining confidentiality of patient/medical staff information.
20. Performs other duties as assigned.
EDUCATION & TRAINING:
Doctor of Phar! macy from an accredited school of pharmacy is required. Completion of a Pharmacy Practice (PGY-1) Residency is or equivalent experience required.
EXPERIENCE:
At least 5 years clinical hospital experience required. Experience with formulary management and coordination of clinical effectiveness programs required. Leadership experience in planning and setting direction for a department, team or program is preferred.
LICENSE & CERTIFICATION:
Registered Pharmacist in Kentucky. Board certification is desired.
