VNA Nazareth Home Care (VNA), a service of Jewish Hospital & St. Mary’s HealthCare, has provided home care and wellness services to central Kentucky and southern Indiana communities for more than a century. Our innovative services and preventative health programs recognize the needs of our patients and promote the highest quality of life. VNA Nazareth Home Care has a team of registered nurses, social workers, companions, dieticians, pharmacists, aides and physical, occupational and speech therapists. The team provides care for Alzheimer’s disease, diabetes, mental health and ventilator use and offers programs for low vision, stroke management, wound management, better breathing, falls prevention, healthy heart and infusion therapy.
Required Experience:
Assesses, plans, implements, and evaluates nursing care for a designated group of patients with problems related specifically to WOC care, demonstrates effective and efficient consulting; completes and submits required documentation accurately and on time; provides patient, family, and staff teaching; facilitates communication about patients; acts as a consultative resource for other VNA staff; provides instruction and supervision to RN’s. LPN’s, and other professionals and paraprofessionals in providing services to the patient.
Minimum of three years clinical nursing experience required.
Demonstrated ability to function autonomously and as a role model within area of professional expertise. Excellent oral and written communication abilities. Ability to relate to general and professional public, all levels of agency staff, and to patients. Knowledge of community health resources.
Required Education:
Bachelor’s degree in nursing with graduation from an accredited Wound Ostomy and Continence Nursing school required. Certification as WOCN.
