The Pharmacistâs role is to provide safe and compassionate pharmacy care to our patients by efficiently and accurately supplying medications and drug expertise in a collaborative environment. This pharmacist will: 1) work collaboratively with physicians and other members of the healthcare team to deliver safe and efficient medication management that improves patient outcomes 2) provide efficient cost effective patient centered care 3) provide verbal and written consultations concerning medication therapy 4) participates in the educational experience of pharmacy residents and pharmacy students 5) provides education to patients 6) provides education to physicians, nurses, and other members of the healthcare team 7) detect, monitor and reports adverse medication events 8) represents the pharmacy department on applicable teams 9) actively manages medication formulary, including therapeutic interchanges and non-formulary requests 10) provides operational oversight to the pharmacy including dispensing, compounding and supervising non-professional personnel. Work involves non-standardized tasks requiring professional judgment. It is expected that the Pharmacist shall exercise leadership in all matters related to the medication use process for neonate/infant (birth to 1 year), pediatric (1-12 years), adolescent (12-18years), adult (18-65), and geriatrics (over 65 years). This position requires a high level of interaction with physicians, nurses, patients, other health care providers, as well as pharmacy management in problem solving and project interaction. The employee is responsible for: supporting and facilitating the QI process through on-going development and monitoring of performance improvement indicators; growth and facilitation of Pharmacy work teams to meet quality, clinical standards of care; assisting the Director to provide effective and efficient use of all resources and for ensuring budget and value proposition targets are met. Required: Graduate of an accredited school of pharmacy. Current pharmacist license in Kentucky or eligible for licensure. Knowledge of applicable medication management regulations, and pharmacy law; Knowledge of pharmacy practice and medication management principles.; Desired: Residency trained, Board Certified Pharmacotherapy Specialist (BCPS), Acute care pharmacy experience, Knowledge of Microsoft Office.
