Clinical Staff Pharmacist REPORTS TO: Director of Pharmacy WORKS CLOSELY WITH: Patient Care Services, Pharmacy staff, Patients and Physicians. SUMMARY OF DUTIES: Assures that primary responsibility of the ph! armacy department is carried out; that is, to see that the correct medication is dispensed to the correct patient by the correct mode of administration at the appropriate time. These tasks take a combination of individual participation and first line supervision of technician staff. Participates in clinical pharmacy programs in accordance with currently established health care standards. CRITICAL DEMANDS: Ability to work independently and handle stress appropriately. Ability to work cooperatively with Baptist Health staff and pharmacy staff. Ability to handle frequent interruptions and adapt to changes in workload and work schedule. Ability to set priorities, make critical decisions, and respond quickly to emergency requests. Ability to manage a diverse workforce. Ability to communicate effectively (orally and in writing). JOB SPECIFIC COMPETENCIES: Principal Duties/Responsibilities 1. Prepares and dispenses medication orders per physician request according to established p! olicies, procedures, and protocols. 2. Interprets medication o! rders (verbal and written) and transcribes to computerized patient profiles accurately. Maintains accurate, complete medication profiles. 3. Compounds and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition products accurately. 4. Issues controlled substances to patient care areas and maintains records as required by law. Ensures safe, appropriate, cost effective medication therapies for patients according to established policies, procedures, and protocols. 1. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug use and dose. 2. Assists with pharmacokinetic drug dosing. 3. Performs dosing for anticoagulation patients and follow-up according to established protocols. Completes necessary documentation. 4. Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed. 5. Reads, extracts, and interprets in! formation in patient charts accurately. 6. Detects and reports suspected adverse drug reactions accurately and in a timely manner. 7. Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic interchange protocols, and promoting rational drug therapy selection. 8. Provides clinical consultation and clarification to practitioners. Communicates effectively and suggests appropriate, cost-effective therapeutic alternatives to medical staff, as needed. 9. Provides accurate, adequate, and timely drug information to the professional staff. 10. Provides drug information to patients and their families, as requested. 11. Documents all clinical activities and interventions accurately and completely. 12. Participates in the quality improvement and medication use review activities of the department. Collects data; conducts quality monitors and inspections; maintains logs, records, and other documentation as assigned. 13. Participates in the development and pr! esentation of orientation, education, and training programs to the phar! macy, medical, nursing, and other staffs. Contributes to the quality and effective operation of the department. 1. Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Participates in the performance appraisal of pharmacy support personnel. 2. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner. 3. Documents review of inspections of all assigned medication storage areas. Resolves any problems that occur. 4. Answers the telephone, identifying self and department. Directs calls to appropriate personnel. Answers requests at the window. 5. Keeps pharmacy areas and equipment clean, neat, and well organized. Maintains competence required for current job title/position. 1. Maintains current pharmacist licensure. 2. Maintains CE hours as required by law and for job title/position. Clinical Staff Pharmacist II designation requires 20 ho! urs of CE yearly, 5 hours to be in one area of study. 3. Attends orientation, education, and training programs. Reviews literature and other materials pertinent to the practice of pharmacy. 4. Completes all competence/skills assessment requirements as appropriate for job title/position. 5. Age Appropriate Skills and Knowledge: Demonstrates the knowledge and skills to provide care appropriate to the newborn (0 - 28 days), infant (29 days - 1 year), child (1 - 12 years), adolescent (13 - 18 years), adult (19 - 59 years) and geriatric (over 59 years) patient including principles of growth and development over the life span of the patient. Possesses the ability to assess data reflective of the patient's status, interprets the appropriate information needed to identify each patient's requirements relative to age-specific needs and provides the care needed as described in departmental policies and procedures.
EDUCATION: B.S. or Pharm D. degree from an accredited univer! sity required. Must be licensed or eligible for licensure by the Kentuc! ky Board of Pharmacy. EXPERIENCE: Hospital experience preferred, but not required. (1 year pharmacy experience required for BS graduates, no additional experience required for Pharm D graduates)
