VNA Nazareth Home Care (VNA), a service of Jewish Hospital & St. Mary’s HealthCare, has provided home care and wellness services to central Kentucky and southern Indiana communities for more than a century. Our innovative services and preventative health programs recognize the needs of our patients and promote the highest quality of life. VNA Nazareth Home Care has a team of registered nurses, social workers, companions, dieticians, pharmacists, aides and physical, occupational and speech therapists. The team provides care for Alzheimer’s disease, diabetes, mental health and ventilator use and offers programs for low vision, stroke management, wound management, better breathing, falls prevention, healthy heart and infusion therapy.
Required Experience:
Directs case management and clinical operations for multidisciplinary geographic team(s); assigns field staff to patient care duties; manages the human resources and the professional expertise of assigned area to coordinate and provide consistent appropriate patient care, including documentation requirements; fiscal management as related to patient care and established budget; additional responsibilities as assigned; assisting with performance improvement/quality assurance standards.
Minimum of three years clinical nursing experience, including supervisory experience, is required. Experience in home health nursing strongly preferred.
Demonstrated leadership, administrative, and supervisory skills. Excellent oral and written communication skills, including public speaking and presentation. Demonstrated knowledge of educational methods and training techniques for the adult learner. Ability to relate to the general public and all levels of agency staff and patients. Skills with personal computer for word processing and data analysis. Understanding of L&R and JCAHO requirements. Demonstrated understanding of dynamics of case management. Ability to supervise and manage variety of disciplines assigned to the team. Knowledge of Medicare/Medicaid reimbursement regulations and third party payor requirements related to home health operations.
Required Education:
Bachelor’s degree in nursing required. Kentucky Registered Nurse License.
Acceptable equivalency:
Associates degree in nursing along with a minimum of six years clinical nursing experience, including supervisory experience.
