The Medication Safety Officer provides a leadership role in formulating policy, practices, and promoting medication safety in all aspects of the medication management process. Develops and coordinates medication safety processes and initiatives to ensure safe and appropriate medication use processes that support optimal patient care. Partners with pharmacy, nursing, medical staff, and other health care professionals to standardize, implement, measure, and improve medication use processes that are compliant with regulatory and accrediting agencies. Oversees the reporting of medication events, assuring they are appropriately evaluated and improvements implemented. Serves as an educator of pharmacy residents, providing these trainees with direct experience in designing, implementing, and maintaining safe medication-use systems. Coordinates Medication Safety Committee activities relating to medication safety and pharmacy compliance. Ensures that all activities are consistent with overall hospital objectives. The position demonstrates competence to provide pharmaceutical care for neonatal (birth-discharge), pediatric (birth-11 years), adolescent (12-18), adult (19-65) and geriatric (66-over) patients.

Job Duties:
Ensures that medication use processes are evidence-based, designed in relation to safety, efficacy, and efficiency. Authors and assists with implementation of medication error reduction strategies in collaboration with all departments and all patient care areas. All information related to medication error reduction will be housed within the Patient Safety Organization as defined by the organization.

Applies the science of safety and human factors as safe medication practices are implemented. For example, assessment of medication use compliance will have a focus on quality indicators. Recommends improvement strategies regarding issues identified and provides assistance for action plan implementation.

Assists in the design of the Pharmacy Department and Nursing orientation plans to ensure initial and ongoing professional competency with the objective of promoting safe medication practices. Serves as a mentor to pharmacy staff, aiding in the development of their job-related skills and verifies maintenance of professional competency and compliance with standards.

Communicates with medical, nursing, pharmacy and ancillary staff in a positive and effective manner. Communicates and collaborates with hospital leaders and staff to ensure timely and accurate exchange of information across the organization.

Plans, develops and implements or modifies medication use processes to meet new or unmet needs. Collaborates with management team to resolve all identified problems and adopt preventative actions. Seeks external funding sources as needs arise.

Participates in the development and implementation medication use processes that uphold accrediting body standards and that are in alignment with the organization's mission, values and strategic plan.

Collaborates with pharmacy management team and provides over-sight in the planning, development and implementation of departmental quality improvement efforts to ensure positive outcomes.

Participates or provides oversight of participation in hospital standing and ad-hoc groups concerned with medication utilization and safety to define present and future needs and to assist in problem resolution.

Provides monthly and/or periodic reports containing qualitative and/or quantitative data.

Oversees, develops and participates in educational endeavors, specific to Medication Safety provided to medical, nursing, pharmacy, and ancillary staff.

Supports the mission of the Department of Pharmacy in the training and guidance of pharmacy students and residents. Serves as preceptor in the area of specialty. Attends and participates in education-related committee meetings. Ensures that evaluations are appropriate and that all accrediting standards are met.

Develops positive relationships in satisfying the needs of internal and external stakeholders. Works to promote compliance with established Medication Safety policies within the department of Pharmacy and the Medication Safety Committee and is an active member of the Patient Safety Committee.

Provides advice and guidance to medical staff and other clinical providers on the provision of evidence-based medication therapies and protocols with regards to age-specific populations.

Mentors staff in providing information to patients and patients' family members on safe and effective medication use.

Performs other duties as assigned.

EDUCATION & TRAINING:
Doctor of Pharmacy from an accredited school of pharmacy is preferred. Completion of a Pharmacy Practice (PGY-1) Residency or equivalent experience is required.

EXPERIENCE:
At least 5 years operational/clinical hospital experience preferred. Knowledge of Six Sigma, Quality Improvement strategies, and the compliance and regulatory issues relative to hospital pharmacy practice is preferred. Leadership experience and highly effective written and oral communication skills are preferred. Hospital Pharmacy Informatics preferred.

LICENSE & CERTIFICATION:
Registered Pharmacist in Kentucky. Board certification and project management certification desired.
